FAS requests for faculty leave due December 3

All ladder members of the Faculty of Arts and Sciences wishing to request a leave for academic year 2013-2014 (or part thereof) or calendar year 2014 should complete the Request for Faculty Leave form and submit it to the department or program chair by December 3, 2012. University policies regarding faculty leaves are described in detail in the Faculty Handbook; any other questions regarding a faculty member's eligibility for leave should be directed to the cognizant provost.
