Annual Faculty Activity Report (FAR) 2010

Attached below is the annual Faculty Activity Report form to be submitted by ladder and other full-time teaching faculty in the Faculty of Arts and Sciences.  It is also available on the web at: http://www.yale.edu/fas/html/far.html .  This form covers service, teaching, and research for the calendar year 2010 and is due in January.  The form is used by chairs and the FAS Steering Committee in the annual salary review that begins early in the spring term.

Once again this year, the form is a Word file that can be filled out electronically or on paper.  Please complete those parts of the form that pertain to you, and send the completed form as an e-mail attachment to your department chair by January 14, 2011.  Department chairs will then submit all reports electronically to the Provost’s Office using Sharepoint.

This is the final year we will use this Word file for the Faculty Activity Report.  By this time next year, we will have adopted a user friendly on-line faculty profile system for the submission of Faculty Activity Reports as well as for creating and updating CVs and departmental web pages.
